This program focuses on employing  “whatever it takes” approach to assist clients with transitioning from homelessness to permanent housing. Clients served in this program are homeless, high utilizers of public services and may have a chronic mental illness or physical disability. 

The program is funded by the Sacramento County Department of Human Assistance,

CSH-FSRP program currently has two intensive case managers who are serving 40 clients. Case management services may range from highly intensive individualized support to less intensive support for activities related to acquiring and maintaining housing, and associated supportive services.

Clients are referred to the program directly from the Department of Human Assistance.
