What is FSRP?

The Flexible Supportive Re-Housing Program (FSRP) focuses on employing “whatever it takes” approach to assist clients with transitioning from being unhoused to obtaining permanent housing. Clients served in this program are identified as unhoused, high utilizers of public services and may have chronic mental health challenges and/or a physical disability.  This program consists of case management services (CMS) and housing services (Property Related Tenant Services-PRTS) for the Flexible Housing Pool.

What are Case Management Services and Property Related Tenant Services?

CMS and PRTS consists of (but are not limited to):

  • The housing First model

  • Housing Support Services

  • Housing Assessment Plans

  • Case Conferencing

  • Multi-Disciplinary Team Meetings (MDTs)

  • Home Inspections

  • Program Exit Planning
